We will file insurance claims. Co-pays and deductibles are due when services are rendered and any outstanding balances that are not covered by the patients insurance are the responsibility of the patient. Services performed without insurance coverage are due when rendered unless prior payment arrangements have been made. In the event of insurance coverage, it is our office policy to bill your insurance carrier although you are responsible for the entire balance. Since our office is not a party to the agreement with you and you insurance company, it is not our policy to contact carriers to establish why they have not paid or question the amount of payment. We cannot render services on the assumption that all charges will be paid by your insurance company. Rare is the insurance company that covers 100% of everything.We accept Medicare, Medicaid Bluecross/Blueshield, AARP, Humana, Cigna, United Healthcare, Evercare, Aetna and Many more types of insurance.
Scheduling and consultation: Once you have filled out the Facility Contract form, it’s as easy as 1, 2, 3!
1. Watch for symptoms of poor skin health. These may include itching, dry skin, abnormal growth, rash, wounds, bruising or anything else that seem unusual.
2. Fax over a face sheet and order form to (813) 792-7895.
3. We will come to you to treat your residents at their bedside.
